Your consult and care plan do the motivational work, the contemplation and preparation. We pick up at the two stages a short appointment cannot reach: action and maintenance, where intention either becomes a habit or quietly dies. We don't try to motivate people who aren't ready. We help the ones who are make it stick.

"Exercise more" fails because it has no time and no place. Every habit in the plan attaches to a cue the patient already has, after waking, with the kettle, before bed, so the day itself does the remembering. Specific plans beat willpower, and the evidence on this is decades deep.
Week one is deliberately tiny: two habits to build, two to ease off. Early wins build the belief that change is possible, and that belief predicts long-term adherence better than motivation does. Capacity grows only when consistency has earned it, so the plan never outruns the patient.
Each card carries a plain-language reason tied to their condition: why strength training for their insulin resistance, why morning light for their cycle. Patients who understand the mechanism keep going when the novelty wears off. Instruction alone does not survive week three.
Maintenance fails at the first stumble, so we designed for the stumble. No streaks to break. Flare mode swaps the plan to gentler versions on hard days, and a flare day still counts as showing up. Hard weeks hold steady instead of resetting. A lapse stays a lapse instead of becoming a collapse.
