01 · What it actually is

A silent pressure, building quietly

Blood pressure is the force of blood pushing against your artery walls. Hypertension is when that force stays too high, over time. It's written as two numbers: the pressure when your heart beats (systolic), over the pressure when it rests (diastolic).

The hard part is that it usually has no symptoms. You can feel completely fine while it quietly strains your heart, arteries and kidneys. That's why it's worth understanding, not because of how it feels today, but because of what it's slowly building toward. And here's the hopeful half: for most people, the way they live has genuine leverage over the reading.

Primary means there's no single cause, but a mix of genetics, age, and daily life. That last part is where you have real influence.

02 · How it works

The high-alert chain behind the reading

Your body has two modes. A fight-or-flight mode (the sympathetic nervous system) that speeds you up for action, and a rest-and-restore mode that calms you down to recover. You're meant to move fluidly between them. When you're under threat, fight-or-flight does exactly what raises pressure: it speeds the heart, makes it pump harder, and tightens the blood vessels. Useful for a sprint from danger. A problem when it never switches off.

For many people with high blood pressure, the body is stuck in that high-alert state: chronic stress, poor sleep, no real recovery. Science calls this sympathetic overdrive, and it's well established as something that both starts and sustains raised pressure. Here's the chain most people are never shown.

03 · Where it sits in your two dials

We measure two things separately: your drive (how activated you are) and your reserves (how much you have in the tank to recover). High blood pressure tends to show up as one particular pattern.

STUDY 01
In a controlled feeding trial, an eating pattern rich in vegetables, fruit and low-fat dairy (the DASH pattern) lowered systolic blood pressure by around 11 mmHg in people with hypertension, a drop comparable to what a first blood pressure medication achieves.

Blood pressure has a body-clock rhythm: it should dip at night during sleep. People whose pressure doesn't dip have worse outcomes, and aligning treatment to that rhythm cut cardiovascular events by 45% in a large trial. Timing and sleep are genuine blood-pressure levers.
